THE WORKSHOP

Have you ever thought about what might happen if you needed to be admitted to a hospital and didn’t have the capacity to make decisions regarding your medical care for some reason? Do you have views or preferences about what medical treatments you would choose to accept or decline that you would want known?

Advance care planning (ACP) is a process that supports you at any age or stage of health in understanding and sharing your  personal values, life goals, and preferences regarding current and future medical care. The goal of ACP is to help ensure that you receive medical care that is consistent with your values, goals, and preferences.

Advanced Care Planning week was 20-26th March 2023 and the CoVision Health Team would like to invite you to attend our second online workshop to learn a bit more about the process and how you can prepare and document your wishes in your own ACP.
